General Description of Duties: The primary duty of the Nurse-LPN is to assist with administering medications, training of staff and provide direct care to the individuals as needed for the health and safety of the individual on the DD Waiver, in accordance with the DDSD Standards. Will be required to adhere to policies and procedures required by the State and DDSD to include, but not limited to: assessments, healthcare reports, health tracking, healthcare plans (HCPs) and medical emergency response plans (MERPs).

Education and Experience: Certification as a licensed practical nurse (LPN) in the state of New Mexico required. One (1) year experience working with disabled individuals, including those with severe/profound disabilities strongly preferred. Experience with feeding tubes (jejunostomy or gastrostomy) also preferred.

Other Requirements: Must pass pre-employment drug and TB screen, and Agency, Federal and State Department of Health background investigations. Must maintain Registered Nursing License in the State of New Mexico. Must be able to successfully complete training courses and pass written competency examinations mandated by the New Mexico Department of Health DDSD, which are presented in English. Must have own, reliable transportation and an insurable driving record. Must possess a valid NM driver's license and proof of auto liability insurance. Must be able to lift 50lbs. occasionally, bend, stoop, reach above shoulder(s), balance, push, pull frequently. Must be able to work non-scheduled hours as needed as well as have sufficient mobility to visit various sites without prior notice. Adelante is a drug free workplace.
